After-School Electives Registration Form

Dear Parents:

We are pleased to be able to offer an enrichment program for your children.  The activities have been chosen to balance intellectual, physical, small motor and concentration

We have been working to meet the needs and interests of all your children and hope you, as parents, will be actively involved in signing your children up for activities so no child will be disappointed because the activity of his choice had to be cancelled.

Guidelines for Activity Registration:

1. Fill out the attached registration form separately for each child.
2. In case an activity is filled by the time we receive your form, please list any alternate choice. DO NOT SEND CHECKS FOR THESE ALTERNATE CHOICES.
3. Send the following forms and checks to our office at 340 Quaker Ridge Road:
   1. Registration form
   2. Separate check for each activity made out to the Hudson Country Montessori School

Please return these forms promptly.  We will make every effort to see that your child gets into the activity of his/her choice, however, since the activities are limited, registration forms will be accepted on a FIRST COME, FIRST SERVE basis.

PLEASE NOTE: If there is not sufficient enrollment for any activity, it will be cancelled.

There will be transportation between 1200 North Avenue and 340 Quaker Ridge Road for these activities.
